Pursuant to Pennsylvania Right to Know Act, I hereby request the following records:

Attention Borough of Etna Freedom of Information Officer:

- Please provide the Borough of Etna Fire Department report for 15 Robertson Place, Etna, PA 15223 (Parcel ID : 0221-H-00104-0000-00), property listed in the Allegheny County records as owned by HALAPCHUK, KATHLEEN WELSH.

- Please provide the police report and / or incident report and all the officers’ names who responded to the fire.

- Please provide a copy of the Building Permit application and all the documents and attachments filed with the application, subsequent to the fire.

- Please provide a copy of the Building Permit issued subsequent to the fire.

- Please provide a copy of the Occupancy Permit issued subsequent to the fire and the description of the number of parking spots to be designated as a result of the new construction.

- Please provide a copy of the Occupancy Permit prior to the fire and the description of the number of parking spots that were required prior to the new construction.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
