Pursuant to the Arizona Public Records Law, I hereby request the following records:

Pursuant to the state open records law Ariz. Rev. Stat. Ann. Secs. 39-121 to 39-122, I write to request access to and a copy of:

A copy of the list of 9-1-1 and/or non-emergency incident call logs between December 1, 1998, and September 1, 2004 for 4420 W 15th St, Yuma, AZ 85364.

All incident reports from and relating to 4420 W 15th St, Yuma, AZ 85364 between December 1, 1998, and September 1, 2004.

I request that you release records for this request on a rolling basis as they become available and in electronic form.

If your agency does not maintain these public records, please let me know who does and include the proper custodian's name and address.

I would request your response within ten (10) business days.
