A copy of the presentation about the "Patient Review and Coordination" program, also commonly referred to as the "PRC" or "Lock-in" program that Phyllis Coolen, RN, MN, presented in Atlanta on January 14, 2009 at the CDC conference, "State Strategies for Preventing Prescription Drug Overdoses."
If available, a recording or transcript of the presentation
All documents created to supplement or accompany the presentation, such as "handouts" that were given to conference participants
All correspondence, including emails and memos, related to the conception, creation, review, presentation and response to this presentation. Please bear in mind that the presentation may have been created for a purpose other than this conference, so a search for responsive documents should date back to whenever the concept/plan was initially discussed.
Copies of any DSHS reports or other documents cited in the presentation
It would be acceptable to me to provide the responsive documents in installments.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or thumb drive if not.
