Pursuant to the Freedom of Information Act, I hereby request the following records:

1. I request that copies of tech issue tickets filed within the months of November 2019 and December 2019 be released for review and examinations of customers' issues resolutions in general.

2. Phone call records, data sharing and sales: Whom have you sold or shared my phone records to (calls, emails, and app usage records)?

3. Provide names of what local federal or private agencies or persons have requested my phone records (calls and otherwise) be made available?

4. I need a copy of my conversation notes with your customer service department--particularly in November 2019 (Concerning suspicions of geo-location data sharing).

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.
