Please produce all complaints regarding Pregnancy Decision Health Centers (EIN: 31-1002913). Relevant complaints are limited to those received on or after Jan 1, 2013.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

Thank you for informing me of the need to amend my request for records related to Pregnancy Decision Health Centers. In order to help determine fees, you should know that I am a member of the general public.

I request a waiver of all fees for this request. Disclosure of the requested information to me is in the public interest because Pregnancy Decision Health Centers seeks to serve those that may be pregnant and/or seeking medical services. It is likely to contribute significantly to public understanding of the operations of this organization as a service provider receiving public funds, and whether one can expect transparent, safe services from Pregnancy Decision Health Centers.
