Please produce all complaints regarding Heartbeat International (EIN: 23-7335592). Relevant complaints are limited to those received on or after April 20, 2013.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you for informing me of the need to amend my request for records related to Heartbeat International In order to help determine fees, you should know that I am a member of the general public.

I request a waiver of all fees for this request. Disclosure of the requested information to me is in the public interest because this organization seeks to serve those that may be pregnant and/or seeking medical services. It is likely to contribute significantly to public understanding of the operations of this organization as a service provider, and whether one can expect transparent, safe services from a Heartbeat International center or one of its local affiliates.
