Pursuant to the Tennessee Public Records Act, I hereby request the following records:
I would like to request copies of any records the Department of Commerce and Insurance's State Board of Funeral Directors and Embalmers has pertaining to the Hickory Hill Funeral Home in Memphis (5270 Knight Arnold Road). I am specifically seeking records related to the discovery of two deceased women in the home in March 2016. The types of records I am interested in include: complaints, site investigations, notices of violation (NOVs), return to compliance documentation, and those related to the suspension of the home's license (motions, complaints, summary judgments, etc.).
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 7 business days, as the statute requires.

My name is Troy Bryant, I am the Associate General for regulatory boards and I am in receipt of your public records request.

We do not have an online portal to pay directly online. However, I can request that our fiscal department give you a call so that you can pay via credit card over the phone.
Once my paralegal has completed your request, I will be in touch with an invoice and can collect the information needed for fiscal’s call and can direct them to call you at a number and time most convenient for you.
