From: Gabrielle Pepin
To Whom It May Concern:
Pursuant to the Tennessee Public Records Act, I hereby request the following records:
Historical licensing data for all licensed child care providers (active and closed) that were open anytime from 1/1/19-present:
• Provider name or ID
• Provider address
• Latest license issue date and expiration date
• First license issue date
• Any additional provider data that your agency collects
If electronic records from before 1/1/19 are available, I’d be happy to collect them as well.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 7 business days, as the statute requires.
Sincerely,
Kent Hoover, a citizen of Tennessee, in coordination with Gabrielle Pepin.

Attached is the data in the format that is available. Our database does not contain the data in the format that you requested. The Public Records Act does not require compiling information to create a record that does not exist.
