I am unable to use the request form in its entirety because whenever I try to copy and paste my request into the form, it sizes the font down to 8, making it unreadable. I have attached it anyway, and have attached a Word document with my original request in it. Are all requests, no matter the government agency, done directly through Raleigh's Public Affairs Department now instead of at each agency?

I originally submitted my request to the Raleigh Police Department on Jan. 18 and didn't receive any kind of response until April 5, when they let me know about this form. I was unable to use the request form in its entirety because whenever I try to copy and paste my request into the form, it sizes the font down to 8, making it unreadable.
