Requested Records To Whom It May Concern:
Pursuant to the Oklahoma Open Records Act, I hereby request the following records:
I am requesting:
1. The Oklahoma State Department of Education's confidentiality agreement required of new hires during onboarding, as well as any revision tracking information and previous revisions of the document.
2. Any logs or existing databases of violations of that agreement, or any reports that summarize how often and the outcomes of investigations into violations of agency confidentiality rules or unauthorized release of information.
If any such responsive materials are duplicative, I would like to reduce the burden of this request and only request log is most comprehensive be released, or if they include the same information whichever is easiest to review and release.
If no such comprehensive logs or databases of such investigations exist, please provide any closing findings or letters of the last 20 completed such investigations.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
