In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

The Bureau of Justice Statistics maintains the Death in Custody Reporting Program (http://www.bjs.gov/index.cfm?ty=dcdetail&iid=243#Publications_and_products). There are a variety of forms that individual jails and prisons may use to submit the information. I'm interested in obtaining the completed forms that were submitted to the BJS.
