All surveys given to students at the start of the school year for the years:

2018, 2019, 2020, 2021

With aggregated student responses for those years as well as free text fields.

Please redact anything that might identify a student or parent individually.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you for the reply.

I'm sorry for the confusion: the surveys I am referring to are administered and created by the district and administered only at the very beginning of the school year prior to students entering class.

For example, a survey was administered this year to incoming junior high students online that asked what pronouns the student preferred and if their parents were aware.

This is different than the Panorama survey that is administered after the start of the school year.

Attached are four documents with the questions and responses to the surveys you have requested that were available for students to complete at the beginning of the 2018, 2019, 2020 and 2021 school years. The survey questions appear at the top of the columns on the first three pages of each of the four documents.

We have redacted personally identifiable student information ([R.C. 149.43(A)(1)(v); R.C. 3319.321; 20 U.S.C. 1232g] including, but not limited to “information, alone or in combination, is linked or linkable to a specific student that would allow a reasonable person in the school community, who does not have personal knowledge of the relevant circumstances, to identify the student with reasonable certainty.” 34 CFR 99.3.
