Pursuant to the Minnesota Government Data Practices Act, I hereby request the following records:

All available annual reports and other documentation for Steve Rummler Hope Network, also called The Steve Rummler Memorial Foundation, FED UP! Rally, Steve Rummler Hope Foundation, Physicians for Responsible Opioid Prescribing (PROP), The FED UP! Coalition, and SRHN. FEDERAL ID# 452903444. The foundation became a 501(c)3 in 2011, so there should be documents going back to at least March, 2011.
I do not need certified copies.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
