From: Corlyn Voorhees

To Whom It May Concern:

Pursuant to the Massachusetts Public Records Law, M.G.L. c.66, §10, I hereby request the following records:

-Guides/instructions for how MyTimesheet works. Some of these materials are referenced in the MyTimesheet-related letter to providers (“To make this transition as smooth as possible, we will also provide videos, manuals, and other tools.”) posted online here: https://www.mass.gov/files/documents/2017/06/zu/evv-pca-consumer-letter.pdf
-Complaints, comments, and other feedback received from outside the department relating to EVV. If this feedback is summarized you may instead provide that summary rather than individual emails, etc.
-A copy of the contract(s) and agreement(s) under which the MyTimesheet software and services are developed, deployed, and managed, as well as any reports or agreements related to privacy and security implementations of the program provided by the vendor.
-Any cost estimates or budgets that cover running the MyTimesheet service (i.e. personnel needed, data storage costs, etc.), whether generated internally by the agency or provided by the vendor.
-Any cost estimates or budgets for the timesheet services (i.e. personnel needed, data storage costs, etc.) used prior to implementing MyTimesheet. Please limit this request to budgets and estimates generated after 2014.
-Any process or staff guidance for securely storing client information (i.e. locations where services were administered, etc.). This also applies to contracts with outside organizations if these services are being outsourced, as well as the results of any security audits performed in regard to protecting this data.

I also request that, if appropriate, fees be waived as we believe this request is in the public interest, as suggested but not stipulated by the recommendations of the Massachusetts Supervisor of Public Records. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

I expect the request to be filled in an accessible format, including for screen readers, which provide text-to-speech for persons unable to read print. Files that are not accessible to screen readers include, for example, .pdf image files as well as physical documents.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days, as the statute requires.

Sincerely,

Corlyn Voorhees
