The code set attached has an exclusion/inclusion criteria for type II diabetes, is that particular criteria desired for the request? Also, please note that the diagnosis codes that will be used will be the ICD-10 versions.

Can you send over the ICD 10 codes for peripheral arterial disease? Please see the attached for the ICD 10 codes I was able to determine for diabetes. You could also provide the ICD 10 code list for diabetes if you have particular codes in mind. However, if you find any concerns or questions or would like to make any modifications please feel free to respond.
