Pursuant to the Kentucky Open Records Act, I hereby request the following records:
Historical licensing data for all licensed child care providers (active and closed) that were open anytime from 1/1/19-present:
• Provider name or ID
• Provider address
• Latest license issue date and expiration date
• First license issue date
• Any additional provider data that your agency collects
If electronic records from before 1/1/19 are available, I’d be happy to collect them as well.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Enter your computer log in name along with your password. After entering this information, go to the orange box to open email. Highlight the records at bottom of email and it will bring a screen to download the record. The records will appear in your download file on your computer.
