Pursuant to the Maryland Public Information Act, I hereby request the following records:
Full case documentation and autopsy report for Terrance Watts (DOB 12/12/91) who died in policy custody on June 4, 2018. This should include, but not be limited to; police response report, CAD data from 911 call, autopsy report, witness statements, etc.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 calendar days, as the statute requires.

We are in receipt of your PIA in which you seek information regarding Terrance Watts. Specifically you have requested the following:
Full case documentation/police report/witness statements
Autopsy report
CAD data from 911 call
The retention period regarding CAD data has passed, therefor, there is no CAD data available for this incident. This included the 911 call.
You must request a copy of the autopsy report from the Office of the Chief Medical Examiner in Baltimore.
The incident report is available for this matter. The City of Frederick charges $5.00 to obtain a copy of a police report. Payment must be received before the materials are released. To obtain the report, please remit a check or money order in the amount of $5.00 ( we are unable to accept electronic payment or payment via credit card) made payable to "The City of Frederick" and mail it to:
