Pursuant to RCW Ch. 42.56 (Public Records Act), I hereby request the following records:

Please provide me with all records pertaining to the shooting committed by Dennis Clark III at the Pinewood Village Apartments on 4/21/2013. The records I am requesting include but are not limited to incident reports and discharged round reports.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

On October 6, 2016, I advised that the report was ready to be release. I indicated that the report was too large to email and asked how you would like to receive the report. You replied with your mailing address.

Again on October 6, 2016, I advised that the mail had already ran for the day and that the CD would go out in Friday's (10/07) mail.

I am unsure why you have sent this follow up request. Have you not yet received the CD which was mailed to the address listed below?

On May 1, 2019, the Federal Way Police Department received your fax requesting follow up information on PDA 16-113.

According to our records, responsive documents were mailed to you on October 6th, and PDA 16-113 was therefore closed.

I have assigned this request, a new PDA number 19-071. A copy of the report is provided to you on the City's FTP site. You can upload the files along with the redaction long.
