To Whom It May Concern:

Pursuant to the Tennessee Public Records Act, I hereby request the following records:

- Incident reports or Crash/Accident reports for the following incidents involving organic peroxide:

1) August 2 incident on Highway 153 involving a FedEx Driver
2) August 3 incident on Interstate 24 where an Averitt truck caught fire
3) Sept. 20 incident where a FedEx box truck leaked organic peroxide near the Germantown Road off-ramp on Interstate 24
4) Oct. 9 incident on W. MLK Boulevard and Riverfront Parkway where a tractor trailer overturned, leaking organic peroxide.

If it is your position that responsive records exist but that those records (or portions of those records) are exempt from disclosures, please identify the records that are being withheld and state the basis for the denial for each record being withheld. In addition, please provide the nonexempt portions of the records. I am seeking each record in its entirety. Accordingly, please do not redact portions of any records as “non-responsive,” “out of scope,” or the like. Furthermore, I am requesting the entire document containing the responsive information, including any and all information in the document that the agency may consider “non-responsive.”

I request that any records produced in response to this request be provided in electronic form wherever possible.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
