Privacy Waiver

I’ve never done a privacy waiver before. Now I am doing a request for the records of a deceased CIA officer and I want to add a privacy waiver from a family member. What’s the best way to proceed? Is there a template for privacy waivers that I could give to the family member? Does it have to be notarized? Can I attach it to my request? tx.
