Pursuant to Wisconsin's Open Records act, I hereby request the following records:

–A list of every hospital, clinic, or other institution that provides sexual assault forensic exams administered by a certified Sexual Assault Nurse Examiner (SANE).

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I would request your response within ten (10) business days.

The Wisconsin Department of Health Services does not maintain the records you are seeking. I recommend you contact the Wisconsin Department of Justice.

As provided in Wis. Stat. § 19.37(1), you may seek review of the Department's response to your request either by petitioning a court for a writ of mandamus to order the Department of Health Services to release the records you seek, or you may make a written request to the County District Attorney's Office or the State Attorney General to file a writ of mandamus requesting a court to order the release of the records you seek.
