1) Provider name or ID
2) Provider address
3) Latest license issue date and expiration date
4) First license issue date
5) Enrollment, including dates on which enrollment was recorded (by child age groups if available)
6) Capacity, including dates on which capacity was recorded (by child age groups if available)
7) Prices, including dates on which prices were recorded (by child age groups if available)
8) Quality Rating and Improvement System (QRIS) scores, including dates on which scores were recorded
9) Any additional provider data that your agency collects

Attached please find a spreadsheet containing records that are responsive to your request, except for enrollment, price, and any additional provider data. I have not made any redactions or deletions to these records.

The Department of Children and Families (DCF) does not collect enrollment or price data from child care providers; therefore, DCF does not possess any records that are responsive to these requests.
Your last request for "[a]ny additional provider data that your agency collects" is denied as not sufficiently limited as to subject matter, as required by Wis.
