Any and all hiring and personnel records for Dr. Leonard "Len" Paulozzi, for any paid, elected, appointed or volunteer position he has held in your agency. I do not have specific dates of his employment, but I know he last worked as a DOH employee at least 24 years ago, so records may be limited. It is possibly that he may have taken special assignments or acted in an advisory or consulting capacity more recently, as he was a frequent collaborator with Washington state agencies in efforts to research and combat the opioid epidemic.
These records should include, but are not limited to:
• Job/Volunteer position applications
• Financial and conflict of interest disclosures
• Resumes and CVs
• Letters concerning offers, hire, promotion, disciplinary proceedings and/or dismissals
• Documentation of commendations, recommendations and rewards
• Training records
• Contracts for services
• Salary and bonus records
• Disciplinary actions and/or investigations and/or decisions
• Letters of resignation

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
