The data record layout, data frame, or any equivalent data framework used by this agency to document maternal deaths. Materials responsive to this portion of this request should include reference to the data elements collected by this department and should not include any personal information. Please feel free to redact any personally-identifying information.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

This is a follow-up to our October 24th telephone conversation regarding your Freedom of Information Act request for maternal death data. I have attached a sample of the VDH Division of Vital Records' current Certificate of Death form. The pregnancy questions highlighted on the death certificate is how the maternal death data is collected.
