This is a request under the Freedom of Information Act. I hereby request the following records:

Records regarding Tommy Thompson's role in securing a contract for Symphony Corp. while he was secretary of the Department of Health and Human Services. I understand Symphony Corp received a contract in 2004 which has since been renewed. The contract has to do with Quality Written Correspondence Monitoring (QWCM) and Quality Call Monitoring (QCM).

I previously requested this information in 2008 and was assigned the following reference number: C09FOI0014M (KS). In 2010, I reiterated my interest in receiving this information in a fax sent to Kimberly Sessions.

To reiterate my request:

1) which employee/s from Symphony Corp. performed that work? Please provide documentation that would show who worked on the contract.

2) What competition, if any, was there for the contract? Please provide a list or documentation identifying any other bidders.

3) what role did then-Secretary tommy thompason have in the solicitation, negotiation or award of the contract? Please provide any documentation showing any participation by Mr. Thompson in the contract.

4) Were there any ethics waivers signed related to this contract? If so please provide copies of them to me.

5) Have there been any assessments/audits/reviews of the performance of the contractor since this contract was awarded in 2004? If so, I am requesting copies of those records.

6) Who is the main point of contact at Symphony corp for the contract?

7) Have there been any challenges from unsuccessful bidders (if any) to the contract? If so, please provide copies of those challenges.

8) Please provide contact information for Mr. Glenn Kodani so I can ask him some more detailed questions about what these contracts entail.

I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.
