This is a request under the Freedom of Information Act. I hereby request the following records:

Documents pertaining to the following:
-Any investigations into TK Potable Diving, Pittsburg Tank and Tower, or Braintree (MA) Public Works, specifically in regards to water tanks.
-Any records relating to the drowning death of David Scott while he was in the employ of TK Potable Diving, working for Pittsburg Tank and Tower, which was in turn contracted by Braintree Department of Public Works to construct their Lincoln Heights water tower. These documents should include any that make reference to safety violations or other workplace violations. The records should also include emails, memos and other forms of communication, if possible.
-Any safety requirements or policies regarding commercial diving, and tank diving in particular.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
