To Whom It May Concern:

Pursuant to the Freedom of Information Act, I hereby request the following records:

All materials and communications (sent and received) regarding Master Sergeant Brian Charles Riley, who was pronounced dead on December 17, 2019 at his off-base housing (105 22nd ST. NW, Minot, ND 58703). Please include all communications with any Air Force personnel whether at Minot Air Force Base, Minot, ND, the Pentagon in Washington, D.C. or any government entity, regarding his medical records.

Please consider as responsive the following materials and communications related to MSgt Riley: files, office notes, regular and "special" delivery" mail, phone calls, memorandums, texts, emails.

This FOIA Request is being sent by the Mother and Primary Next of Kin of MSgt Brian Charles Riley. I have requested his medical records 7 times using Standard Form 180 (SF-180) to your Processing Center with his Death Certificate each time. I have been denied my rights.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

Sincerely,

Diane L Junk

On July 7, 2021, Mary D Castillo, CtR, USAF at the AF Str Processing Center in San Antonio, TX sent a letter addressed to no specific person via Muck Rock. It stated that the STR Records records request could be obtained through Minot AFB-5th Bomb Wing, and she provided an address.

My FOIA, submitted on February 21, 2021, was NOT about obtaining my son's STR records.

The FOIA was for "All Materials and Communications, sent or received by the STR Processing Center regarding Master Sergeant Brian Charles Riley". Among the items I expect to receive via this FOIA are my 10 Submissions of SF180, my calls to and from San Antonio STR Records Center and their calls, faxes, emails, and voice messages to and from the USAF at Minot AFB, Minot ND.

Thank you,
The Mother of MSgt Brian Charles Riley (Birth Certificate proof attached)

This is a response to the 21 Oct 2021 letter, via the USPS, not Muckrock, regarding this FOIA Request. The letter is from Katie A Blackwell, CTR, USAF at the AF STR Processing Center in San Antonio, TX 78217

AGAIN, I REITERATE, THIS FOIA IS NOT FOR MY SON'S MEDICAL RECORDS.

I am uploading a scan regarding that letter. Ms Blackwell states: "While this facility is in possession of the veteran's physical Service Treatment Record (STR) (outpatient medical and dental records), we are unable to fulfill your request at this time for the following reason(s):" (reason is omitted because it is irrelevant)

AGAIN, I REITERATE, THIS FOIA IS NOT FOR MY SON'S MEDICAL RECORDS.

IN CLOSING, SHE CONTRADICTS HER EARLIER STATEMENT (ABOVE) BY SAYING "The medical and dental records for this veteran can be requested at Minot AFB". I WAS TOLD MONTHS AGO THAT BRIAN'S MEDICAL RECORDS ARE NOT A MINOT AND HAVE BEEN TRANSFERRED TO THE NATIONAL ARCHIVES - eVET RECORDS. WHO IS LYING TO WHO? THAT IS WHY I WANT THE COMMUNICATION RECORDS BETWEEN STR CENTER AND MINOT AFB.
