Pursuant to the Freedom of Information Act, I hereby request the following records:

The Official Military Personnel File (OMPF) for Nicholas Mongillo including Form DD-214, all Fitness Report and Counseling forms NAVPERS Form 1610/2, NAVPERS form 1610/1, the Background Summary (NAVPERS Form 1640/32), Cumulative Record of Training (NAVPERS Form 5300/16), history of assignments NAVPERS 1070/605, Navy Training History NAVPERS form 601-4, all military decoration and full award citations, all military education/training courses, military service number.

Mr Nicholas Mongillo claims retired naval aviator. approximate service dates 1986 to 2013 active duty. Claims numerous awards including silver star, meritorious service medal, Air Medal, navy commendation medal. Served during operation DESERT STORM. piloted F/A-18 Hornet during aviation career, graduated from Navy Strike Fighter Tactics Instructor program TOP GUN and later commanded the Pacific Missile Range Facility – Kauai in 2012/2013.
Specifically request copy of history of assignments NAVPERS form 1070/605, copy of silver star citation, all commendation medal citations and all unit award citations with redactions as determined by FOIA officer.

PLEASE NOTE: I represent the general public and am not next of kin of this servicemember, and thus do not know the military service number. Please process my request for this servicemember's OMPF and other records under the Freedom of Information Act’s relevant provisions.
I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be processed by a representative of the news media/press.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
