This is a request under the Freedom of Information Act. I hereby request the following records:

The report summarizing findings of the US national investigation into the strike on the Doctors Without Borders (MSF) Trauma Center in Kunduz City, Afghanistan , on October 3, 2015, as well as all annexes, appendices, digests, abstracts or other accompanying materials.

Per a statement by General John F. Campbell, Commander, Resolute Support and U.S. Forces -Afghanistan given on November 251h, 2015, at Resolute Support headquarters, Kabul, Afghanistan: "I received the report of the US national investigation into the strike on the Doctors Without Borders (MSF) Trauma Center in Kunduz City, Afghanistan , on October 3, 2015. [....] It was important that the officers investigating the incident have the requisite seniority and independence to conduct a thorough and unbiased inquiry. For that reason, I requested an outside investigative team. USCENTCOM supported my request , and sent an Army major general independent of U.S. Forces-Afghanistan to lead the investigation, assisted by two brigadier generals- one from the Army and one from the Air Force- also from outside my command. The report included specific findings relating to systems, processes and personnel and I have already approved some of the findings and recommendations."

As this request is submitted in my capacity as a journalist who has covered the US military extensively, I assert that my appropriate status for the purpose of fees is "media requester." Furthermore, as the requested documents are of considerable public interest and will released in full to the public, I request that all fees be waived.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.
