This is a request under the Freedom of Information Act. I hereby request the following records:

All minutes from any "Epi-Chiefs" teleconference meeting hosted by the GEIS division of the Armed Forces Health Surveillance Center (AFHSC), now a component of the Defense Health Agency (DHA), from July 1, 2012 to the date of processing of this request, which reference the antimalarial drug mefloquine (also known as Lariam).

I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

Thank you for your Freedom of Information Act (FOIA) Request, received by the Defense Health Agency (DHA) on September 23, 2014.

Your request seeks " All minutes from any "Epi-Chiefs" teleconference meeting hosted by the GEIS division of the Armed Forces Health Surveillance Center (AFHSC), now a component of the Defense Health Agency (DHA), from July 1, 2012 to the date of processing of this request, which reference the antimalarial drug mefloquine (also known as Lariam)". These records would be maintained under the U.S. Army and so you would have to submit a FOIA request to them, as DHA does not maintain US Army records. The AFHSC falls under the Army, the DoD Directive 6490.02E designates the Secretary of the Army as the DoD Executive Agent for the AFHSC. Please see attachment.
