This is a request under the Freedom of Information Act. I hereby request the following records:
A copy of the Army’s Personnel Files (OMPF) for a former servicemember named MICHAEL WILLIAM KRZYZEWSKI (DATE OF BIRTH: 02/13/1947). Mr. Krzyzewski was born in Chicago, Illinois. He attended United States Military Academy at West Point between 1965 to 1969, received his commission upon graduating from West Point in 1969, and eventually achieved the final rank of Captain when he left the Army in 1974. The requested records include performance reports, job assessments, disciplinary records, training received, discharge papers, and any other releasable records.
The public interest in Michael William Krzyzewski, who is a nationally famous men's basketball coach at Duke University, should favor disclosure of many of these records, particularly in relation to any disciplinary records, should those records exist.
Please be sure to review and release any segregable information, even if certain documents or portions of documents are withheld.
PLEASE NOTE: I am not a next of kin of this servicemember, and thus do not know Mr. Krzyzewski’s Social Security Number. Because of this, I am ONLY requesting records releasable under the Freedom of Information Act. Please process my request for this servicemember's OMPF and other records under the Freedom of Information Act’s relevant provisions. I am not seeking any records that would be releasable under the Privacy Act.
I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be processed by a representative of the news media/press. This request is made in the process of news gathering and not for commercial usage.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.
