This is a request under the Freedom of Information Act. I hereby request the following records:
The Official Military Personnel File (OMPF) including all Fitness Report and Counseling forms (NAVPERS Form 1610/2), the most recent Cumulative Record Of Training (NAVPERS Form 5300/16), the Background Summary (NAVPERS Form 1640/32), Officer Personnel Action Requests (NAVPERS Form 1301/85) and Form DD-214, for James David Vance, a former member of the United States Marine Corps.
Vance was born on August 2, 1984 and enlisted in the Marine Corps in or around 2002, after graduating from high school.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
