Pursuant to the Freedom of Information Act, I hereby request the following records:

The Official Military Personnel File (OMPF) for RANDY EDWIN RHODES including Form DD-214, all Fitness Report and Counseling forms NAVPERS Form 1610/2 - 1610/1, Enlisted performance evaluation reports NAVPERS Form 1616-1, the Background Summary (NAVPERS Form 1640/32), Cumulative Record of Training (NAVPERS Form 5300/16), history of assignments NAVPERS 1070/605, Navy Training History NAVPERS form 601-4, all military decoration and citations, military training courses.
Specifically request copy of history of assignments NAVPERS form 1070/605, Navy commendation medal citations, navy achievement medal, defense meritorious service medal citation and joint service commendation medal citations if possible with redactions.
Mr Randy Rhodes claims numerous decorations/awards, rank O-6 Commander, approximate service dates 1971 to 1998, birth date unknown. This request is submitted to corroborate claim to be a U.S. military veteran. NPRC claims unable to locate the OMPF file.

Through his exceptional professional competence, sound leadership and dedication to duty, an unprecedented unit readiness posture was achieved. On numerous occasions, he presented innovative contributions which impacted directly on the increased capability of the unit to respond to highly specialized world-wide operations. Additionally, he displayed superior talents and abilities in the organization scheduling and supervision of numerous highly dangerous and sensitive training evolutions, His individual initiative and forcefulness of purpose characterized those attributes critical to the dangerous and exacting demands placed on him. His diligent efforts, resourcefulness and personal sacrifice inspired all who observed him and contributed significantly to the successful accomplishment of the command's mission. By his exceptional professional ability and selfless devotion to duty, Lieutenant Rhodes reflects great credit upon himself and the United States Naval Service.”

LT Rhodes demonstrated the highest level of professional acumen and extraordinary dedication to duty by systematically and meticulously drafting the Naval Special Warfare Group TWO Master Training Plan (MTP) published April 1988. LT Rhodes displayed foresight, drive and a sense of purpose typically seen in more senior officers. Through countless hours of painstaking analysis, LT Rhodes developed a thorough, logical and concise MTP. A major achievement for this group, the MTP is a lasting contribution to Naval Special Warfare readiness. LT Rhodes reasoned, systematic approach and unfailing determination brought order to a fragmented program that had been only partially effective. This achievement reflects great credit upon LT Rhodes and is in keeping with the highest highest traditions of the United States Naval Service.”
