The Official Military Personnel File (OMPF) for RANDY EDWIN RHODES including Form DD-214, all Fitness Report and Counseling forms NAVPERS Form 1610/2 - 1610/1, Enlisted performance evaluation reports NAVPERS Form 1616-1, the Background Summary (NAVPERS Form 1640/32), Cumulative Record of Training (NAVPERS Form 5300/16), history of assignments NAVPERS 1070/605, Navy Training History NAVPERS form 601-4, all military decoration and citations, military training courses.
Specifically request copy of history of assignments NAVPERS form 1070/605, Navy commendation medal citations, navy achievement medal, defense meritorious service medal citation and joint service commendation medal citations if possible with redactions.
Mr Randy Rhodes claims numerous decorations/awards, rank O-6 Commander, approximate service dates 1971 to 1998, birth date unknown. This request is submitted to corroborate claim to be a U.S. military veteran. NPRC claims unable to locate the OMPF file.

The Freedom of Information Act (FOIA) request concerning Navy veteran Randy
Edwin Rhodes that you submitted to the FOIA office at the Navy Personnel
Command in Millington, TN has been referred by their FOIA officer, David

A major achievement for this group, the MTP is a lasting contribution to Naval Special Warfare readiness. LT Rhodes reasoned, systematic approach and unfailing determination brought order to a fragmented program that had been only partially effective.
