This is a request under the Freedom of Information Act. I hereby request the following records:

Any and all files on Douglas DeWitt Bazata 17th Feb 1911 - July 14th 1999.

Bazata, who served with the United States Office of Strategic Services behind enemy lines in France in World War II and later became an artist, died on July 14th 1999 in his home in Chevy Chase, Maryland. He was 88.

Please conduct a search of the Central Records System, including but not limited to the Electronic Surveillance (ELSUR) Indices, the Microphone Surveillance (MISUR) Indices, the Physical Surveillance (FISUR) Indices, and the Technical Surveillance (TESUR) Indices.

Proof of death is not required it was widely reported at the time:
