Records relating to the DEA investigation of Costco pharmacies, ultimately leading to a $11.75 million settlement agreement with the government in January 2017.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

This is a clarification my earlier request of December 3, 2017, and in response to your request for clarification of December 13, 2017.
Please retrieve any and all Reports of Investigations (DEA Form 6, or DEA-6) involving two Costco pharmacies defined below. Also, please retrieve any Administrative Inspection Warrants or Search Warrants pertaining to the pharmacies below.
The investigating offices at which these records may be found are DEA Diversion Groups and/or Field Offices in Seattle (WA), Los Angeles (CA), and Sacramento (CA).
The two pharmacies for which information is requested are:
1. One located in Everett, Washington, with a place of business of 802 134th St. SW. This pharmacy has a Provider Identification number of 4931918.
2. One "central fill" located in West Sacramento, California, whose DEA license was recently revoked.
