Pursuant to the Freedom of Information Act, I hereby request the following records:
Every Form CMS-2567 issued between 2017 and 2022 for the following hospitals listed in the attached document.
For your convenience, I’ve included the CMS Certification Number for each hospital, when available, and the corresponding address.
I’m requesting expedited processing due to a “compelling need” for the public, particularly the residents living in these regions, to know about the quality of hospitals in their area.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

The analyst that performed the document search tells me there are three (3) empty files marked no results for Dickenson Community Hospital, Russell County Hospital, and Smyth Community Hospital. There were no files meeting the request located. She inadvertently forgot to remove them from the zip. Apologies for any inconvenience or confusion.

Hello,
After looking through the responsive documents, I noticed we didn't receive a CMS-2567 form that we know was issued for Holston Valley Medical Center following a March 10, 2020 survey. I've attached the 2567 form below.
Knowing this inquiry didn't produce the document mentioned above, I'm wondering if I've made my request too narrow or included language that would have missed other CMS-2567 forms.
I expected more responses, particularly from the larger hospitals listed in the original table I sent. But I noticed there are no responsive documents/CMS2567 forms for Johnson City Medical Center, Bristol Regional Medical Center or Holston Valley Medical Center.
I'm hoping you can help me understand the discrepancy, especially as it relates to the attached form. Thanks.

Hello,
There seems to be some confusion. The Philadelphia Office ONLY handles requests regarding the states of Pennsylvania, Delaware, Maryland, Virginia, West Virginia and DC. Regarding the VA hospitals all 2567s located were released.
Holston Valley Medical Center, Bristol Regional Medical Center and the others appear to be in Tennessee. This office does not have information regarding facilities in TN. I believe the Atlanta Office covers TN. Hope this helps.
