CDC Reports, complaints, etc. re: Attention Deficit Disorder Biological Research, 1990 - 2015

To Whom It May Concern:

This is a request under the Freedom of Information Act. I hereby request the following records:

Any and all reports, complaints, investigations, files, and any other materials regarding Attention Deficit Disorder research from the period from 1990 through 2015. Please include all segregable data, including, but not limited to, the product name, the brand name (if applicable), ingredients, the nature of the complaint, etc.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes. This request is being made by a member of the news media and should be categorized appropriately.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
