Pursuant to the Freedom of Information Act, I hereby request the following records:
Any documents pertaining to collaboration between the Centers for Disease Control and Prevention (CDC) and the production of the 1995 film "Outbreak," directed by Wolfgang Petersen. Such documents may include, but are not limited to, script notes; production assistance agreements; and all records of communications between employees of the CDC and members of the cast, crew, and production company of "Outbreak," including memos, notes of meetings and conference calls, emails, and other forms of communication.
I would also like to know if the CDC provided any form of support or technical advice whatsoever to the production of "Outbreak" or to any member of the cast or crew, or if the production or any member of the cast or crew sought assistance from the CDC in connection with the production of the film.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

I hereby request that any fees, search fees and otherwise, be waived, since, quoting the HHS, website, these documents "will contribute significantly to the public’s understanding of the operations or activities of the government," and my request "is not predominantly commercial in nature." I am the Director of Research Operations for a 501(c)(3) non-profit organization called The Decoding Culture Foundation. These documents will be made available to the public for free on our website and will not be used for commercial purposes.
