Pursuant to the Freedom of Information Act, I hereby request the following records:

A copy of the purchase order or something showing the product name, identifier, quantity, price, and seller for:
• soap for washing hand located in the room containing toilet and sink for patient use;
• soap for washing body, face, and/or hair given to the patient using the shower, or to be located in the room containing shower;
• soap for washing washing hand of worker exposed to patient and/or to infectious material which might be near the sink located in the corridor;
• soap for washing hand of patient food handler.

These soaps are intended to be used at Walter Reed National Military Medical Center. Some soaps may come with the admission kit and/or amenity kit.

The date of sending the purchase order, purchasing soap, or the agreement happening is from January 1, 2018 to October 1, 2019. I am requesting only 1 soap buying record for each soap purchased during those times. For example, if 4 purchase order contains the same soap, I only need 1 purchase order.

I am requesting records for soaps that are normally purchased for those uses instead of all soaps. Creating record that shows the information instead of searching for the existing record is fine.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.
