Southwest Food Excellence SFE

To Whom It May Concern:

Pursuant to the Texas Public Information Act, I hereby request the following records:

1. Food vendor contract that Grand Prairie ISD has with Southwest Food Service Excellence (SFE)

2. Minutes and Agenda's along with any school board votes to approve anything that pertains to Southwest Food Service Excellence (SFE)

3. Any and all conflict of interest forms on file that pertains to Southwest Food Service Excellence (SFE) Please include any school board members and/or GPISD staff members with any conflict of interest forms on file.

4. Please include the salary of Tara Clear (Daughter of School Board Trustee Terry Brooks) who is employed with Southwest Food Service Excellence along with dates of employment.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
