To Whom It May Concern:

Pursuant to the Texas Public Information Act, I hereby request the following records:

I am requesting data on all complaints submitted to the Department of Corrections' Inmate Grievance Program between January 1, 2022 and December 31, 2022.

Specifically, I am requesting a spreadsheet that contains, among other relevant variables, the grievance identification number, prison, grievance contents or description, date of the submission, date of acknowledgement, date of decision, name of staff member who reviewed the submission, and the outcome of the submission.

If the Department does not maintain such a spreadsheet, then I request the raw copies of the reports and any other relevant supporting documentation.

Please note that I am not requesting any identifying information about any incarcerated individuals.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes. In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
