Federal hospital surveys

To Whom It May Concern:

Pursuant to the Tennessee Public Records Act, I hereby request the following records:

Copies of all Form CMS-2567 Statement of Deficiencies reports pertaining to the following mental health hospitals in Tennessee. These federal surveys were performed by the Department of Mental Health & Substance Abuse Services on behalf of the Centers for Medicare and Medicaid Services.

-Crestwyn Behavioral Health Hospital in Memphis: all records between April 21, 2016 and August 1, 2022

-Erlanger Behavioral Health in Chattanooga: all records between June 1, 2018 and August 1, 2022

-Ascension Saint Thomas Behavioral Health in Nashville: all records between March 1, 2021 and August 1, 2022

-East Tennessee Behavioral Health in Knoxville: all records between July 1, 2022 and August 1, 2022

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
