4. 193 Broadway ("Louie's, LLC" and any other name listed with this location)

5. 13 Elm Street, Somerville, MA ("Aguacate Verde, Silvia DeLaSota and any other name listed with this location)

6. 10 Tyler Street, Somerville ("Artisan's Asylum, Gul Cavalcanti, and any other name listed with this location)

7. 253 Washington Street, Somerville ("Casa B, Alberto Cabre, Angelina Jocovich, and any other name listed with this location)

8. 255 Washington Street, Somerville, MA 02143 ("Ronnarong Thai restaurant" and any other name listed with this location)

9. 321 Washington Street, Somerville, MA 02143 (the "Washington Street Art Center" and any other name listed with this location)

Please provide these as actual copies, not scanned-image copies, of the originals. We expect that you will charge us .10 per Permit page copy.
