To Whom It May Concern:
Pursuant to the California Public Records Act, I hereby request the following records:
Shasta County Jail Policies and Procedures including any formal or informal policies regarding booking, medication administration, use of force, mental health crisis, deescalation, medical care including hospital transport and emergency medical care.
I am also requesting locations of video cameras at the Shasta County Jail and all video camera footage of Teddy Woodrow Abbie taken while he was in custody on September 11 and 12 2019.
Please also provide booking information for the September 11th booking of Teddy Woodrow Abbie, his initial medical visit after booking, and any logs or documentation associated with his time incarcerated on September 11, 12, 2019.
Finally, provide the names, titles, and any history of complaints or disciplinary action against any Shasta County Jail personnel who were actors or witnesses in the death of Mr. Abby including those involved in restraining him, observing him or those restraining or moving him, monitoring him after he was moved, or any other activity surrounding the death of Mr. Teddy Abbie while in custody.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 calendar days, as the statute requires.
