Pursuant to the New York Freedom of Information Law, I hereby request the following records:
A copy of the burial, cremation or transit permit for a deceased person, as named below:
Pamela Cafritz, distinguished by a Social Security Number that begins with the first 5 digits 214-58, date of birth November 12, 1959 and reported date of death November 7, 2016. Proof of death is attached to this message in the form of PDF obituary from FindAGrave.com
The records would have been submitted to the Clerk as registrar of vital records.
Please note: this request is not for a death certificate (as described under Public Health Law § 4174), but for a Burial, Transit, or Cremation Permit --separate documents, required under a different section of law, specifically Public Health Law § 4145. A 2009 advisory opinion of the Committee on Open Government found that Burial, Transit, or Cremation Permits are records covered by the Freedom of Information Law. Please find that decision attached.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
