Pursuant to the California's Sunshine Amendment (Cal. Const. Art. I, § 3(b)), I hereby request the following records:

This is a request for all documents and reports that relate to the suicide of Walter Graham Suggs Jr. on 9 June 1964 at 2570 Great Highway, San Francisco, CA 94116.

I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 calendar days, as the statute requires.

Please note that this is a one-time courtesy. For any future requests, the fee approved by the San Francisco Board of Supervisors for a Medical Examiner’s Report at the time of the request, must be paid prior to any report processing. A waiver will not be considered.

I am writing in response to your Freedom of Information Request inquiry dated 4/30/2015.

The Departmental Retention policy regarding the requested articles outlines and permits disposition of the requested articles once a Cause and Manner of Death are determined. At the time a case is closed, and the manner is determined, the articles or notes are released to the family member or to the person to whom the letter is addressed.

The San Francisco Office of the Chief Medical Examiner is unable to meet your request because the article you are seeking is no longer in our possession.

This case (1964-1200) was mannered a Suicide and was finalized at the conclusion of the examination. The article requested is no longer available, and was released or destroyed in compliance with Medical Examiner’s Office Departmental Policy.
