Official military medical records

Pursuant to the Texas Public Information Act, I hereby request the following records:

I'm requesting my military medical records for VA benefits. I did not receive these during discharge processing. Cook, Brittany N. DOB 04/26/1989

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

I am also following up on my previous requested with the completed SF-180 Form for requesting Military Records. Please see file attached to receive all medical records from AIT & BAMC on Fort Sam Houston during 03-2012 up to 10-2012 (Discharged in 09-2012).
