Pursuant to the Government Records Access and Management Act, I hereby request the following records:
Any and all police reports and related records concerning Dr. Barbara Snow, who on February 11, 2006 entered a relative's house with a baseball bat and damaged property.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

All of the information I have about the incident is from a complaint filed against Dr. Snow with her licensing oversight board. You can view the complaint here, on page 7:
Information regarding the address where the incident took place is redacted, but the licensing board certainly has it. I am not sure if they would be able to tell you. I'm unable to locate a date of birth for her, but again the Division of Occupational and Professional Licensing has it.
If this information is not sufficient then I'll modify request to a copy of all police reports from February 11, 2006.

Thank you for this information, it's very helpful. I'd like to request any and all records related to the petition for a certificate of eligibility for expungement. According to Section 103 of the Utah Expungement Act:
"(1) The petitioner shall apply to the bureau for a certificate of eligibility for expungement and pay the application fee established by the department.
(2) Once the eligibility process is complete, the bureau shall notify the petitioner."
I am seeking the application, the notification from the department, and any other records related to the expungement of the February 11, 2006 police report involving Barbara Snow.
This is a follow to your email in regards to the request for any and all records related to the petition for a certificate of eligibility for expungement:
That request would need to be directed to the BCI Office. We would have just gotten a court ordered that had been signed by the judge to carry out the expungement. After we complete the process legally there is no record after that. The fact that we were able to even find that it was expunged on that date is a miracle. You can refer to https://bci.utah.gov/expungements/expungements-faq/ . I think that this would be the best way to contact them.
