Pursuant to the Oklahoma Open Records Act, I hereby request the following records:

Local news reported on a crash on September 14 on the I-40 eastbound ramp near Mustang Road involving a tractor trailer.

Therefore, I request any crash reports or incident reports, photos, videos, or other notes or emails sent about this incident. The files will be used in the course of reporting for Motherboard, VICE Media's Science and Technology site. www.motherboard.vice.com

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

DPS requires Open Records Requests be submitted on the Open Records Request form, which can be found at www.oklahoma.gov/dps<http://www.oklahoma.gov/dps>. Click on More, then Forms. You may attach additional pages or a letter for clarification. Please re-submit your request.

All requests for audio/video must be mailed separately to the Dash Cam Video Production Unit on the Dash Cam Video/*55 Audio Request form. This form is on our website as well.
