Pursuant to the North Carolina Public Records Law, I hereby request the following records:

A copy of all North Carolina amusement device inspection records and injury reports from 2010-the date this request is filed, as required by Article 14B-Amusement Device Safety Act of North Carolina. This includes records from the "North Carolina Safety Amusement Star Program Amusement Device Safety Inspection" form and any other forms which collect information regarding amusement device inspections, and injuries reported as required by 95-111.10 of the Amusement Device Safety Act of North Carolina.

If available, I also request written documentation needed to understand the data, such as a data dictionary and/or code book.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

If possible, please provide the data in a machine-readable format, preferably as a CSV file (not a PDF).

Angelica: After taking a closer look at your request, I wanted to get back to you to let you know that your request in its current form is voluminous. You have specifically requested “all North Carolina amusement device inspection records and injury reports” from 2010 to present. The Elevator and Amusement Device Bureau conducts roughly 7,000 regular amusement device inspections per year, so inspection reports from 2010 to present would result in thousands of documents. In addition, incident reports are separate reports that are one to four pages per incident. We have gathered the incident reports only for 2014 and it is approximately fifty (50) pages. Please also be advised that only those Elevator incident reports in which an injured party was transported are required to be reported to NCDOL. This request would also take multiple weeks to fulfill and there would be extensive fees associated with it. We are currently working with very limited staff due to the pandemic, and this may increase the time needed to gather all of these documents. In addition, we do not keep these reports in any type of data file. Please let me know if we can work with you to narrow the scope of this request.
